JACKSON, Wyo. — Network of Care is a free online platform that features a health and social services directory, health data tracking, and a health topic library. Established in 2017 by St. John’s Health and Teton County Health Department as part of the Healthy Teton County initiative, the web portal allows users one point of access to comprehensive community services.
The Network of Care web platform is available 24/7 365 days a year, in English and Spanish. It can be accessed through the Teton County Health Department website or by visiting https://publichealth.networkofcare.org/teton-wy.
Explore the Network of Care health service directory through key word searches or exploring categories that feature local healthcare providers and agencies. Search by provider, speciality, insurance, etc.

- Mental health and counseling
- Food and nutrition
- Support groups
- Family support services
- Physical therapy and more!
Users can create a profile to save provider listings and health provider information. The site also includes a “share” feature so that users can share information with loved ones and caregivers.
Providers interested in being added to the Service Provider Directory can submit their details here for review by the Teton County Health Department in accordance with the Network of Care Inclusion policy.
