JACKSON, Wyo. — St. John’s Health, Jackson, WY, is pleased to introduce the providers of the Sage Living care team: Nurse Practitioner Laura-Lee Berrett, DNP and Medical Director Kathleen Vanier, MD.

Laura-Lee Berrett began her nursing career in 2006 as a long-term care nurse with St. John’s Health. Since then, she has served in nursing roles across the western United States, including various roles in senior care. After completing her Doctor of Nursing Practice (DNP), Berrett is now an on-site Nurse Practitioner at Sage Living. In this role, Berrett is in constant communication with residents and the care partners and loved ones closest to them.

From preventative care and routine medical care to more acute medical needs, she is devoted to providing comprehensive care for residents. Additionally, she provides in-person evaluations and directs transfers to the hospital when a higher level of care is necessary. “I am often the first person to learn of a resident not feeling well and can assess them immediately,” said Berrett. “I then report my findings and suggested plan of care to their primary care provider.” Berrett highlights the value of her relationships with residents. “I provide companionship through my relationships,” she said. “The better I know the residents, the easier it is to detect subtle changes in their health that may be early warning signs of bigger issues.”

Sage Living Medical Director Dr. Kathleen Vanier is a seasoned provider to the unique needs of seniors. “I love visiting with the residents both unofficially, chatting with them in the dining room or sitting with them in the common areas, and officially, performing medical exams when necessary at their bedsides,” she said. Dr. Vanier has been working with seniors for over 20 years, as an internal medicine hospitalist and outpatient provider. “I have never before been privileged to spend time with and observe patients in their homes as I do at Sage Living,” she remarked. “It is a joy to work in this setting with these residents and their families.”

Dr. Vanier attended medical school at Johns Hopkins University School of Medicine. She serves as associate director of the St. John’s Health Connections Geriatric Assessment program, a consultative program with various experts that help individuals and families assess an elder person’s physical, cognitive, and emotional health as it pertains to functional independence and quality of life.

Sage Living embodies culture change in long-term care, embracing the practice that the resident directs their own life and collaborates with the team on the care that they want. Dr. Vanier continues this respectful philosophy in her practice at Sage Living as its medical director, working closely with nursing, therapy, care coordinators, other physicians, pharmacists, and the hospital with the goal of constantly improving quality of care through evidence-based practices. “Sage Living is one of the most wonderful places I have worked. Sage Living is a place where the residents can shine and thrive. The staff is amazingly caring and dedicated to the residents. They want the residents to have the best quality of life. This means making Sage Living a home for the residents; it is a place where they feel comfortable, respected, loved, and well-cared for.”
